Please print and fill out all of the new patient forms and bring them with you to your first OR first visit of this calendar year. Note that if you have not been seen in our office in the past 12 months you will need to fill out the new patient forms. ALSO VERY IMPORTANT IS THAT YOU WILL NEED TO BE SURE TO BRING YOUR CURRENT INSURANCE CARD AND PICTURE INDENTIFICATION TO YOUR APPOINTMENT WITH YOU. WE CANNOT PROVIDE SERVICES WITHOUT YOUR CURRENT CARD AND CURRENT PERSONAL DATA IN OUR SYSTEM. Please arrive 10 minutes early.
